PIC stated they had a problem with homeless people living in the back lot behind the facility and when they moved out all the insects and pests moved over to the facility. Instructed PIC that only approved pest control companies can lay down pesticides in a E & D facility. PIC discarded can at time of inspection. Correct By: Corrected At Time Of Inspection

PIC stated that she was not aware that they were not allowed to have a y-valve attached to the faucet. PIC removed the y-valve at time of inspection. Corrected At Time Of Inspection Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained Observed an AVB that was corroded, leaking, and damaged. System was connected to a Y-valve for an undetermined amount of time. PIC stated she did not know how long the AVB had been like that.
