Observed a fully detached floor tile and several cracked tiles near the soda bibs for the beverage system. Flooring must be maintained in good repair to be smooth and easy to clean, especially in "high splash" areas. Corrective Action: Correct prior to next routine inspection. 6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Observed an upright single-door freezer that was broken and could not be repaired in the back prep area. Unnecessary items and equipment shall be removed from the premises in a timely manner. Corrective Action: Correct prior to next routine inspection.

Observed a leak coming from the ceiling in the back room directly above the mop sink at time of inspection. Ceiling panels visibly waterlogged. PIC stated that there was a work order submitted to address the issue. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
