In cold room, cutting boards deeply scored. Resurface or replace. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and SANITIZED, or discarded if they are not capable of being resurfaced. Cutting surfaces such as cutting boards and blocks that become scratched and scored may be difficult to clean and sanitize. As a result, pathogenic microorganisms transmissible through food may build up or accumulate. These microorganisms may be transferred to foods that are prepared on such surfaces. In the full service refrigerated meat case, observed and unpackaged raw chicken sausage slightly touching an unpackaged raw pork sausage. Maintain separation between different raw animal species with different required minimum cooking temperatures. After discussion with the person in charge, it has been determined that the root cause of the violation was a verification breakdown, which resulted in the violation. The PIC immediately instructed staff to place a barrier/divider between the two different raw animal species. Corrective Action: Corrected at time of inspection.

Ensure adequate separation between raw animal species with different required minimum cooking temperatures. A worker immediately placed a divider between the pans of sausages. Inspector's notes

At the three compartment sink, observed no quaternary ammonium in the third vat. After discussion with the person in charge, it has been determined that the root cause of the violation was training staff and a verification breakdown which resulted in the violation. It was determined that the dispenser was inoperable. Staff will manually add quaternary ammonium sanitizer and verify the concentration using available test strips until the dispenser is repaired and functioning properly. The PIC immediately scheduled repair service for the dispenser. Corrective Action: Corrected at time of inspection.
