-- Observed several damaged/missing floor tiles throughout the establishment: missing tiles underneath fryer station and mop sink as well as damaged floor tiles near 3-compartment sink/prep area. Observed hand sink near grill with open gaps/spaces in sealing to the wall. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Physical facilities must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

-- Paper towels not available for handwashing sink near grill. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. The person in charge (PIC) stated there are no paper towels available to restock hand sinks. A re-inspection will be conducted within 10 days to ensure all hand sinks have a hand drying provision such as paper towels available. Corrective Action: Correct prior to re-inspection.

-- Time as a control documentation not labeled for the following TCS items being held at room temperature on prep table: shredded cheddar cheese, American cheese, muenster cheese, ghost pepper American cheese, Asiago cheese, sliced lettuce, and sliced tomatoes. The PIC stated the items were removed from cold holding and placed on line approximately 2 hours prior to the inspection (10:30 AM). All TCS foods being held under time as a control were properly time stamped at time of inspection. Corrective Action: Corrected at time of inspection.
