RTE food that was contacted by bare hands was discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Project about 90% complete.Correct the following items:1) Install an RPZ backflow preventer on all water lines to combi oven.2) Install an RPZ backflow preventer on all water lines to glass rinsers in bar.3) Re-weld areas of 3-compartment sink that will support rails for racks to dish machine (where rail supports are attached to basins of 3-compartment sink). All welds must be smooth and easy to clean, to meet NSF requirements.4) Seal under counters in kitchen and in bar, including sealing any raw wood and caulking any gaps.5) Install cove base where missing in bar and kitchen. If a cove base is tooled in, ensure it is a minimum 3/8” radius.6) Complete requirements for Other Effective Means of Barrier Protection. Install at minimum 1 insect lights inside dining area, install either fly bait stations or have residual treatment placed outside (both front and back patio), install rodent bait stations (minimum 2 on each front and back patio), have basic crawling insect pest control, create a training for employees, and create a log for staff monitoring of pests.7) Finish peeling all stainless protectant.8) Finish all trim and detail work, including installing corner guards, repairing any minor wall or tile damage, etc.9) Espresso machine must be employee monitored at all times, and customers cannot be sat at espresso machine.
