Discussed prohibited non-approved floor coverings installed in food preparation areas, walk-in refrigerators, warewashing areas, and toilet room areas such as carpeting used in food prep areas, in walk in refrigerators, in warewashing areas in toilet room areas (including handwashing lavatories, toilets, or urinals, and in refuse storage rooms. Floors should be smooth, durable, non-absorbent and easily cleanable. The person in charge removed the carpeted mat at the time of inspection. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with owner, Katie. Construction completed and approved. Inspection rights and inspection report was emailed to KatieAddress the following items:1. Provide 1 inch air gap for the ice machine that is separate from the espresso drainage line2. Provide sneeze guard for the three compartment sink and utensil storage area3. Secure cove base under the food prep sink and be the mopsinkThis establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection conducted with , . At the time of this inspection, this project was about 75% complete. Inspection rights and inspection report was emailed to KatieAddress the following items prior to the next inspection:1. Provide test certification for the ASSE 1013 reduced pressure zone backflow preventer at time of final inspection2. Provide ASSE 1022 dual check valve for the coffee and espresso machine . Vent to face down3. Secure spray faucet on the handwash station to not extend out or make handwash station into food prep sink (indirect plumbed) with faucet not extending beyond 1 inch above the sink flood lip 4. All handwash stations to be 100^F if tempered. All other sinks to be 110^F hot water within 45 secondsProvide hard piping for the espresso machine. No soft piping for drainage lines5. Provide 1 inch air gap for the drainage lines to the floor sink flood lip6. Provide separate water line at the mopsink 7. Provide cove base in the front service area and in mopsink area8. Repair cove base in the restrooms to provide flush transition between the flooring and schulter cove base. Currently area can collect water and debris9. All tile grout and mortar to be flush with tiles10. Provide more light in the restrooms11. Provide covered trash container in the restrooms12. Provide washable ceiling in the dry storage areas and in the mopsink room. Currently no ceiling in the closet and acoustic tiles in the office area 13. Seal around the sinks14. Provide sneeze guard at the three compartment sink and food prep sink15. Seal all cracks, crevices and penetrations including exposed wood under the countertops16. Seal corner cabinet by providing access door or open so opening is smooth and all area is sealedPlease continue with construction according to plans, stipulation letter, and inspections. Please call (602) 402-3147 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No county legal action will result from the inspection.
