Observed employee place sharpie cap in mouth to write on cup, place sharpie in pocket, and continue to prepare drinks without washing hands. Observed employee drop packaged cups on floor, pick up cups and discard exterior wrapping, and rinse hands before proceeding to prepare drinks. Observed employee touch neck and shirt collar then attempt to prepare beverages without washing hands before. Employee properly washed hands at handwash sink at time of inspection. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. Discussed with PIC to retrain staff on proper hand washing procedure and when to wash hands. Also discussed with PIC preventing contamination of writing utensils when in use. PIC had writing utensil sanitized at time of inspection. Corrective Action: Corrected at time of inspection.

Construction has been completed.Most items noted in the last report had been addressed except for sealing the gaps in between the tongue in groove wood walls in service area. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Plumbing inspection was conducted with the superintendent, Shawn L. In-wall stub-out had been completed. Please address the following items prior to the next inspection:1. Provide a water stub-out with angle stop at the mop sink for chemical dispenser.2. Provide a reduced pressure zone (RPZ) assembly (ASSE 1013) at the water connection point of the RO system. Conduits and fittings after the RPZ must be stainless steel or PEX. Do not use copper or brass alloys downstream from the RPZ.3. Provide a RPZ at the water connection point of the mister system, rinsers and blender rinser.4. Provide a vented dual check valve (ASSE 1022) at the water connection points of the coffee/ tea brewers and espresso machines. These are not testable devices, so replace them whenever they leak.
