Observed open packages of sliced pepperoni dated 1/27, precooked sausage crumbles dated 2/15, cream cheese dated 1/3, and sliced ham dated 2/23, in the walk-in cooler. The open TCS foods several days past the discard date; PIC discarded, see embargo form. Discussed 7-day date marking. Day 1 begins the date that a package of TCS food is opened, such as deli meats, or cooked/prepped, such as noodles or proteins. Adding 6 to day one (6+1) determines the discard date. The Food Code requires that TCS foods be dated and discarded 7 days after opening/prep when held at 41F or less to prevent a hazardous amount of Listeria. PIC will retrain employees about date marking and disposition. Corrective Action: Corrected at time of inspection.

Inspection report was emailed to Cody J.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Conducted onsite visit to discuss construction related items with superintendent Cody J.Inspection report was emailed to Cody J.Okay to stock freezer and dry storage.Correct the following items prior to final inspection:1) Relocate the RPZ (ASSE 1013) backflow device so that it is easily accessible for routine maintenance and testing (currently located in the ceiling). Test and certify the backflow device and provide the report. Backflow device must not be installed higher than 5 feet from the finished floor.2) Provide a spill proof vacuum breaker (ASSE 1056) upstream each hose reel. Must be installed at least 12-inches above the highest point of downstream piping. Test and certify the backflow devices and provide the test report. 3) Finish installing the piano hinges to provide access to the floor sinks. 4) Provide a hard pipe for the ice machine and the dish washer. Flex lines are not approved. Drain line must not sit on the ground. Provide a 1-inch air gap above the floor sink. 5) Provide a 3/8” stainless steel cove base to the wall and floor juncture underneath the hood along the entire bottom wall. 6) Provide high temperature silver colored silicone to the gaps of the hood.7) Provide a bead of silicone around the hot wells. Please continue with construction according to plans, stipulation letter, and inspections. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. At the time of this inspection, this project was about 70% complete. ***Per Cody J. the 100-gallon hot water heater is dedicated to the kitchen.Inspection report was emailed to Cody J.Correct/address the following items prior to final inspection:1) Provide an ASSE 1013 RPZ backflow device for EACH combi oven. Provide a hub drain and rigid drain line below the RPZ backflow device and drain to the nearest floor sink.2) Provide an ASSE 1022 dual check valve backflow device upstream of EACH non-carbonated beverage equipment. This incudes the nourishment stations for the coffee brewers. 3) Test and certify all backflow devices and provide the test report.4) Change out the ceiling tiles in the janitor room in the kitchen, restroom in the kitchen, and dry storage room in the kitchen. Tiles must be washable vinyl. Perforated ceiling tiles are not approved. 5) Flash the gaps near the walk-in cooler.6) All floor sinks must be 50% exposed. Cut out the area below the counter tops in the dining area for the water filler station. May provide a piano hinge.7) Ensure all cold holding equipment is on and has an ambient air temperature of 41’F or below.8) Provide 100-110’F of hot water to all hand sinks, and 110’F to all other sink fixtures within 45 seconds.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed.
