To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.No county legal action will result from the inspection. Some pre-existing fixtures and water heater have been replaced or removed. A minor remodel has been opened to finish the items in need of fixing or repair.Ensure the following items are addressed to qualify for a permit:1. Provide a hot water heater 30-gallon (with water heater stand and T&P relief drain) or tankless water heater 5GPM at 60-degree rise.2. Provide hand washing sink with metal splashguards for the front counter area.3. Provide a floor sink in lieu of current floor drain (you may construct an above ground unit).4. Provide indirect drain for the 3-compartment sink (via floor sink or HUB drain/funnel).5. Provide a mop sink with hot and cold water (FRP behind sink).6. Replace the RO system with a filter system.7. Provide a small sink for the water filler station adjacent to 3-comp sink.All other items may be addressed on the remodel permit:1. Provide a sneeze guard at the front counter. 2. Cap off vent from the removed dryer.3. Smooth easy to clean ceiling tiles (may be FRP cut to size).4. Provide missing electrical outlet covers.5. Ensure concrete floors are smooth easy to clean (water must bead off the floor).6. 3-comp sink faucet must not be able to extend into sink basin (lock counterweight).7. Restroom must have self-closing doors (hinges).8. Provide several coats of semi-gloss paint for the underside of counters (exposed wood).9. Seal all gaps, crack, and penetrations (plumbing and electrical).10. Provide missing escutcheon plates under front sink area.Of Note:Ice machine may need to be lifted onto a platform in order to drain, if above ground floor sink is installed.
