Observed some missing floor tiles in front of the deli cooler walk-in and also in front of the 3-compartment sink. Also, some areas of cove base are missing around the mop sink and other adjacent areas. Missing floor tiles and cove base create a condition such that the floor is no longer smooth or easily cleanable. Discussed with person in charge to repair floor and cove base. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. PRIORITY VIOLATION-3-501.16(A)(1), P: Time/Temperature Control for Safety Food, Hot Holding. Observed meatball soup in the hot holding self-serve station that was measured using the probe thermometer with an internal temperature of 121 degrees Fahrenheit. Per PIC soup had been placed in the hot holding station approximately an hour and a half prior to taking temperature. PIC had staff rapidly reheat soup to 165 degrees Fahrenheit and adjusted the dial to a higher temperature on the hot holding station. After discussion with the person in charge, it has been determined that the root cause of the violation was training staff on also routinely taking temperatures at the hot holding station to verify hot holding temperature of at least 135 degrees F.
