Final Construction inspection conducted with operator and Jeff. Construction completed and approved.Inspection report emailed to operator. Please address the following prior to routine inspection:1. Seal all penetrations electrical and plumbing.2. Provide self-closer for the rear entrance.3. Provide FRP behind sink fixtures as discussed.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Plumbing inspection conducted with site superintendent. At the time of this inspection, this project was about 50% complete.Inspection report emailed to Jeff and operator.Please address the following prior to next inspection:1. Provide an NSF commercial grade hand sink with splashguards in the warewash room.2. Provide an NSF commercial grade prep sink with an indirect connection to the floor sink.3. Provide sneeze guard with side panel for the front counter area.4. Provide a copy of the RPZ backflow test report.5. Ensure drain for the pre-filter is braced in a manner that it will keep air gap with the floor sink.6. Extend the FRP behind the 3-comp sink to the floor and do the same for prep and hand sink.7. Ensure restroom is self-closing and shuts all the way.8. Provide smooth transition from prep area to front kitchen area.9. Seal any holes, gaps, or penetrations (plumbing and electrical). 10. Paint any exposed wood with semi-gloss paint (several coats).11. Ensure the ceilings are smooth and easy to clean (kitchen, washroom, restrooms).12. Ensure there are no gaps on the to our bottom of waterproof paneling used.13. Mop sink faucet needs to be replaced with a built-in atmospheric breaker, or secured so it will not go below sink basin flood rim.14. Provide a door sweep for the rear exit.15. Sand around the ice machine floor sink and seal.16. Remove the door of the ice machine room.17. Provide an open-ended toilet seat for the restroom.18. Seal all sink fixtures and toilet seat basin.19. Provide a dipper well or conserve well in the ice cream servicing area.20. Provide a copy of certified food manager certificate. Of Note:Area used for ice cream must be adjacent to the store front hand sink as discussed.Informed operator they must contact local municipality for requirements of grease trap.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 506-6824 for an equipment/construction inspection between 70 - 85% completion of project or if an additional onsite walk-through is needed.
