Conducted hot water test to ensure that sufficient hot water is provided with 50 gallon water heater, per last report. Current installed water heater met demands. If water heater is to be replaced, please provide <75gallon water heater.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Advisory inspection conducted with Mat, Owners, Contractor and Management. Establishment not operating at time of inspection. Under remodel.Remodel permit will be required for new bar, new finishes, new fixtures throughout.Report will be emailed to Mat.Required items that will need to be addressed prior to permit issuance:1. Provide 50 gallon water heater.2. Provide Mop sink with hot and cold water and a mixing faucet with Atmospheric Vacuum breaker.3. Provide a NSF approved food prep sink with hot and cold water indirectly connected to waste a floor sink with 1 inch air gap above finished floor.4. Provide an ASSE 1013 reduced pressure zone assembly upstream of carbonator. If fed by RO system as discussed, then it need to be stainless.5. RP is in place for RO system. Since it is upstream of RO, it may be brass type as discussed.6. RPs will need drain cup drained to floor sink with 1 inch air gap above finished floor.7. Provide undercounter lighting in bar area. Dark FRP and finishes are installed and do not allow for easy visibility for task lighting and cleaning.8. Seal counter at wall juncture with silicone.9. Seal all fixtures at wall junctures.10. Brace all drain lines with permanent bracing.11. Brick veneer that is planned along back bar needs to be replaced with a surface that smooth and easy to clean for the splash area along counter (about 12iches). Discussed different options with owner.12. Provide cove base in back bar area.13. All paint will need be semi or high gloss paint.14. Increase lighting in restrooms.15. Toilet seats in restroom need to be u-shaped.16. Provide small, covered trash can for female restrooms.17. Seal all gaps, cracks and penetrations throughout.No county legal action will result from this inspection.
