Final Construction inspection conducted with Logan . Facility in operation prior to permit approval. RM-11445 required since facility in operation.Inspection rights and inspection report was emailed to Logan and JonThe following items will be continued to be addressed under RM-11445. 1. Provide air curtain for the rollup door. Air curtain must have micro switch to activate once door begins to open along with no deactivation of the air curtain via light switch or on/off button2. Provide ASSE 1022 dual check valve for the hot water dispenser3. Provide pot filler faucet or a scissor faucet for the filling of the dough bowls. If utilizing a pot filler with an atmospheric vacuum breaker, the shut off button must be removed4. All ceiling are to be easy cleanable, durable, non-absorbent and smooth. No exposed areas allowed. Ceiling exposed above equipment in the back portion of the prep area. Water lines are to be troughed. 5. Floor not sealed in multiple areas including the pallet storage areas6. Facility has multiple areas that the wall finishes are easy cleanable, durable, non-absorbent and smooth. All CMU brick must be sealed. Refer to stipulation on requirement of no holes, thickness. FRP paneling does not have frp trim dividers. FRP needs to be sealed with a washable silicone caulk, not an acrylic caulk. Proofer room FRP paneling needs to be adhered to withstand high temperature of 80^F and humidity. Door frame of the proofer room not completely sealed and in disrepair.7. Seal all cracks, crevices and penetrations. Missing cleanout covers, escutcheon rings, cove base FRP paneling connections8. Provide indirect plumbing for the food prep sinkThis establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department.
