Final Construction inspection conducted with site superintendent Tim M. Construction completed and approved for FD-23-05401 (Retail) and FD-23-06108 (Wine Tasting Station).Inspection report was emailed to Tim MCorrect the following items prior to routine inspection:1) Provide the test report for all testable backflow devices. 2) Provide escutcheon plates where missing and silicone the gap between the counter tops in both restrooms. 3) Backflow device for the dishwasher must be visible and easily accessible (no higher than 5 feet from the finished floor).4) Provide a 3/8” cove base at the base of the mop sink basin. Use butyl caulking or a siliconized grout. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Equipment inspection conducted with Site Superintendent Tim M. At the time of this inspection, this project was about 90% complete. Inspection report was emailed to Tim M.Correct/address the following items prior to final inspection on May 20th:1) Provide a visible and audible alarm system for the Liberty pump model 405.2) Provide the correct backflow device at the mop sink for chemical dispensers. Provide an ASSE 1013 RPZ backflow device. Current ASSE 1015 double check valve is not approved for chemical dispenser connections. Test and certify the RPZ backflow device and provide the test report. 3) Cut open an access panel near the toe kick of the cabinets to access the floor sink. Floor sink must be 50% exposed for routine maintenance (wine tasting area). 4) Provide 100-110’F of hot water to the hand sink in the ware washing room and both restroom hand sinks. Note that the insta hot must produce 1gpm at a 60’F rise. Or provide an insta hot for each restroom hand sink.5) Sleeve the flex drain line coming off the dishwasher into a hard pipe. Ensure drain line does not sit on the ground and is elevated with a 1-inch air gap above the floor sink. Maintain slope to drain. 6) Provide FRP to the bottom of the melamine/laminate cabinets where plumbing is installed. 7) Dishwasher must reach 50-100 ppm for the chlorine sanitizer. 8) Ensure all air curtains turn on and off via a micro switch. 9) Provide missing escutcheon plates around pipe penetrations.10) Silicone around the drop-in sinks. 11) Provide the TC of O or C of O from the city.Please continue with construction according to plans, stipulation letter, and inspections.
