Final Construction inspection conducted with Facilities Manager. Construction completed and approved. Owner applied for a Hospital Food Service permit, however operations manager of the facility does not serve highly susceptible populations, such elderly, child or patient care. Facility provides low income housing, education, and meals to residents only. Changed permit type from Hospital Food Service to Boarding Home Class 5.Inspection report was emailed to Garrett HamischComplete the following items by next routine inspection:1. Repair missing tile below hot water heater.2. Ensure chlorine sanitizer concentration at dishwashing machine maintains between 50 -100ppm.3. Ensure quaternary ammonia sanitizer test strips are readily available.A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Equipment inspection conducted with Facilities Manager. At the time of this inspection, this project was about 75% complete. Inspection report was emailed to Michael SmithPlease correct the following items prior to Final Inspection:1. Upgrade current 40 gallon water heater to a 75 gallon water heater. Raise water heater to six inches above floor.2. Reinstall prep sink and drain indirectly to floor sink with a one inch air gap.3. Install Reduced Pressure Principle Zone Assembly (RPZ) backflow preventer between water line source and reverse osmosis (RO) system in dining room. Ensure there are no copper or brass hardware/fittings downstream of RPZ. Provide backflow testing report at final inspection.4. Remove garbage disposal or install a drain trough between garbage disposal and dishwashing machine.5. Provide one inch air gap on chemical line at mop sink.6.
