New owner re-inspection conducted. New food prep sink was installed. Please see if you could zip tie the pipes (Three compartment and food prep sink) together at the floor sink for stability or if you need the space under the sinks pipe the drain line to the wall, then over and forward again to the floor sink.Please re-brace the hub drain under the RO system backflow valve so the cup lines up with the vent on the valve.It is okay to install drying shelves over the prep sink for washed utensils or storage. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. PRIORITY VIOLATION-5-203.14, P: Backflow Prevention Device, When Required - Noted RO system installed to the side of the ice machine Please install a reduced pressure zone backflow valve upstream, install with hub drain and lead the drain to the floor sink. Valve will need to be tested and certified. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained: a). Cut back the drip tray to above the hub drain at the tab, drain, if there is splash then cut the pipe down in a 45 degree or less angle and start the cut above the edge of the hub drain. b). The backflow valve drain lines can or be removed of the backflow valves under the espresso counter, or zip tie to the drain pipe of the espresso machine and follow grade and maintain air gap. c). Please mount the ice machine drain to the espresso machine drain line and keep an air gap at the floor sink. d). Caulk the mop sink at the floor juncture. e). Install an expandable plug for the floor sink next to the prep table in the back room. You could also install a trap seal. f). Three compartment sink has a drip at the center compartment, please looks like a gasket. Correct By: Correct Prior To Reinspection
