Observed the following food items cold holding at internal temperature of 51-53 deg F in a reach in unit that was holding >41 deg F. pizza sauce, provolone, feta, ricotta, fresh mozzarella, pepperoni, Italian sausage, meatballs, sliced chicken, shredded mozzarella, roasted onions and peppers and mushrooms and tomatoes and bruschetta. PIC informed all food items had been cold holding in reach in >48 hours and discarded. Also observed cooked spaghetti cold holding in a plastic container by sauce on cart at internal temperature of 56 deg F. PIC informed the spaghetti had been left on count <2 hours and placed in walk in. Corrective Action: Corrected at time of inspection.

All TCS food items cold holding in unit was discarded or redirected to alternate units. Currently there is no TCS cold holding in unit. Manager will call for repair. Observed spray nozzle attached to hose on mop sink faucet. The faucet at the mop sink has an AVB, which has been under pressure. Had PIC disconnect spray nozzle from the mop sink hose and explained to keep the spray nozzle disconnected when not in use. Discussed with PIC about having the AVB looked at by a plumber since it was under pressure to prevent failure. Explained to PIC that the AVB valve is not designed to be continuously under pressure and that there shouldn't be any obstructions downstream from the AVB valve. This could result in the backflow prevention device to fail. Corrective Action: Corrected at time of inspection.

Observed mop sink AVB valve leaking profusely when the water is turned on. A spray nozzle was attached to the hose on the mop sink at time of inspection, which was under pressure. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to re-inspection on 8/1/2024.

Observed multiple TCS food items (see embargo form for list food items) storage in the prep cooler with an internal temperature range of 57F-60F. Per cook, the TCS food items have been in the prep cooler for over 4 hours. All TCS foods improperly cold held were discarded by cook at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Corrective Action: Corrected at time of inspection.
