Observed nacho cheese in hot holding unit with a temperature of 91*F. Person in charge (PIC) stated that the unit had just been turned on and the cheese was opened yesterday staying in the unit overnight. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. All TCS foods improperly hot held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Observed 3 packages of beef jerky made by Asi Sabe Sonora available for sale. Observed 12 packages of beef jerky made by La Gacha available for sale. Both packages had no inspection stamps. Upon questioning, person in charge (PIC) stated receipts are not easily accessible and could not recall if boxes that the product was received in had inspection stamp. All food must be obtained from an approved source as defined by law. Unapproved beef jerky were true embargoed. Do not remove product from establishment. Obtain receipts by re-inspection date. Corrective Action: Correct prior to re-inspection. Re-inspection conducted with USDA investigator. Beef jerky products determined from unauthorized source and voluntarily destroyed at time of inspection. Corrective Action: Corrected at time of inspection.

Inspection report was emailed to Eduardo.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection conducted with owner, Eduardo. Inspection conducted with RS #1467.At the time of this inspection, this project was about 80% complete. Inspection report was emailed to Eduardo.Prior to final inspection:1. Ensure all drain lines are quick to drain lines. During inspection 3 compartment sink, prep sink and hand wash sink were very slow drain and held water.2. Provide additional refrigeration.3. Seal 3 compartment sink, prep sinks and handwash sinks at wall juncture with silicone and caulk.4. Tighten faucet at mop sink so that it does not extend into basin.5. Provide FRP or water resistant material on clean side of the 3 comp. 6. Paint or finish out dry wall and wood on water heater.7. Provide a u-shaped toilet seat in restroom8. Paint restroom door with semi or high gloss paint.9. Paint door frames in restroom with semi or high gloss paint.10. Paint wood table under cold holding cabinet with semi or high gloss paint.11. Finish installation of dip well. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company.
