PIC stated that a dish machine will be installed in the kitchen, as well as the bar area. ************This establishment has a new owner under the same LLC. Please create a Tyler Account to submit remodel plans.************** Staff provided the contact email.Inspector uploaded pictures of the sink fixtures (see attached files)PIC stated once the remodel is complete, there may be changes to the menu. Please note that the current permit is under a class 3 and may need to be changed to a class 4.

Observed raw hamburger patties holding 44-45F and hot dogs holding 49F in the low drawer refrigerator doors under the oven. PIC stated the hot dogs were placed in the unit from the previous day and everything else was placed in the unit today. PIC discarded the hot dogs and moved all of the other products into working refrigerators. TCS food items must be stored at 41F or below. Corrective Action: Corrected at time of inspection.

Adjust the prep sink counterweight so that the faucet does not go below the flood rim of the prep sink. 3. Finish caulking in any gaps underneath the bar countertop. Remove the tape wrapped around any PVC/ABS drain lines. Provide clamps instead. 4. Provide a hard pipe for the tap system. 5. Seal in the hole to the left side of the walk-in unit. All food items must be cooked in the oven under the type 2 hood. ***Walk-in beer/keg cooler and bar is under RM-11269. Walk-in cooler is installed, but still needs to be connected and turned on.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. PRIORITY VIOLATION-5-202.14, P: Backflow Prevention Device, Design Standard 1) Remove the copper piping downstream of the RPZ backflow device for the soda system. Only PEX, plastic or stainless steel is approved. 2) Provide a hard pipe for the ice machine. Flex lines are not approved. Ensure drain line has a 1-inch air gap between the end of the drain line and the floor sink. Flex line was sitting inside of the floor sink. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained 3) Separate the hand sink and dump sink drain lines at the bar. Separate the prep sink, 3-compartment sink and hand sink drain line in the back of house. 4) Switch out the prep sink faucet for a standard faucet equipped with a hot and cold handle. Prep sink currently only provides hot water. Provide hot and cold water. 5) Switch the hot and cold-water lines for the hand sink next to the prep sink. Hot must be on the left and cold on the right. OC inspection conducted with owner Danielle T. Establishment is not selling any TCS or opened food/drinks to the public. Only unopened non-TCS foods/drinks are being sold to the public. Establishment is in the process of obtaining a new dishwasher, walk-in cooler, new soda system and new equipment for the hood. Hood will require a sign off from the fire marshal/city of Mesa. ***A remodel permit will be created. Owners will need to provide cut sheets for the dish washer and walk-in cooler.***A second remodel will be required for the addition of a new bar. Discussed needing plans and cutsheets for the bar. Bar will be the second phase once the kitchen is approved. Correct the following items prior to final inspection:1) Provide an NSF approved prep sink connected to hot and cold water. Provide a rigid drain that drains to the floor sink. Provide a 1-inch air gap above the floor sink. Do not tie drain line into the hand sink drain lines. 2) Provide two additional NSF approved hand sinks. One in the ware wash area, and one on the other side of the wall next to the prep sink. Provide splash guards to the hand sinks if within 24-inches of prep sink or food items/equipment. 3) Current hood is a type 2 hood. Contact the fire marshall/city of Mesa for hood requirements and provide the approval. A type 1 hood is required for grease laden foods. 4) Provide hot water to all sink fixtures. Hot water must reach all sink fixtures within 45 seconds. All hand sinks must have a hot water temperature of 100-110’F and all other sinks must have a hot water temperature of 110’F or above. 5) Provide/relocate RPZ (ASSE 1013) backflow device for the soda system once installed. Ensure there is no copper or brass lines/fittings downstream of the RPZ and soda equipment. Test and certify the backflow device and provide the test report.6) Ensure each chemical dispenser has an internal air gap (ASSE 1055A).7) Provide a consumer advisory statement with asterisks next to any undercooked hamburger items. 8) Replace any burnt out lights throughout the establishment. Provide any missing light covers. All light covers must be shielded. 9) Adjust the flow rate for the 3-compartment sink.10) Provide cut sheets for the walk-in cooler and dishwasher.11) Provide any cleanout covers/escutcheon plates where missing.12) Ensure walk-in cooler has a 3/8” stainless steel cove base inside and outside of the walk-in.13) Flash the sides of the walk-in cooler.14) Provide dry wall, FRP, or stainless steel to any exposed CMU.15) Repair air curtain as needed.
