Please address the following items prior to the routine inspection: 1) Provide a door sweep to the back exterior door. 2) Provide new black foam insulation for the walk-in cooler condenser line to provide a smooth and easily cleanable finish. 3) Re-bracket and re-caulk the sink fixtures throughout the facility, as needed, with silicone to the walls. 4) Provide the missing dome grate at the floor sink.5) Provide a self-closing device /hinge for the restroom door. 6) Fill all wall holes, as needed, with silicone. 7) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility with silicone. 8) Replace the damaged floor tiles near the back exterior door. 9) Repaint the small dry storage room with semi-gloss paint to provide a smooth and easily cleanable finish. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services.
