OC Reinspection - Permit Approved. Food Catering Class 5.Upon re-inspection, all violations have been corrected.Conducted inspection with Food and Beverage Director. Director stated catering events will be offsite and typically food storage equipment and transportation is rented from a 2nd party vendor. Director provided written standard operating procedure for catering operations.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. For catered events, advised person in charge (PIC) to obtain a self contained portable handwashing sink that is 9X9X5 hand sink, 5 gallon potable water tank, 15% greater capacity waste water tank, hot and cold water (a heating element is required), and a minimum flow rate of ½ gallon per minute; or a temporary hand wash station shall include the following at a minimum: 5 gallon insulated container with a free/continuous flowing spigot for potable water; water at a temperature of at least 38°c (100°f) and a 5.75 gallon container w/lid to hold wastewater.
