Inspector's notes

Observed a container of salsa on a prep table with an internal temperature of 109*F. Per PIC the salsa was cooked to temperature, removed from heat, and placed on a surface to cool 3hrs prior to time of inspection. Educated PIC that all cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. Corrected at time of inspection.

New owner follow up inspection. RO system was installed and installer now added a reduced pressure zone upstream.Valve was tested and certified. There is some copper between the backflow and the filter system of the RO system which is approved.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Follow up inspection with ownership. The following items will need to be corrected:1). Install a Reduced Pressure Zone backflow valve upstream of the RO system between the copper and the RO system. Install backflow valve with hub drain assembly and lead indirect to a floor sink.1). a Reduced Pressure Zone backflow valve (ASSE1022) will require testing and certification after installation.2). Remove the brass and copper from the water storage tank. There should be no copper or brass downstream of the RO system.3). Please remove the P trap from the ice machine drain line. Please follow gravity flow to the floor sink.4). Cut back the drain line at the mop sink and provide an air gap.5). Please install an expandable plug or trap seals in the floor sinks that do not receive water.6). Remove or close off the front of the wood under the ice machine. You can install legs or wheels . Store is okay to stock and train. New permit inspection walk through with the owner.This establishment is not participating in the award program at this time and had 0 Priority, 1 Priority Foundation violations on this inspection.Restaurant is not open and is going through a face lift. Operator will install at existing water and sewer connections:1). A hand wash sink in the front service area with hot and cold water and a p-trap.2). Install a food prep sink in the front or back room, sink will have hot and cold water and will need to drain to a floor sink. Keep an air gap.3). Clean the AC vents over the three compartment sink.4). Install a water filter, no backflow valve required unless you change to an RO system.5). Repair the self closer on the back door.6). Hood and whole back room will professionally cleaned.When installing drain and water lines, make sure to run them along the wall, properly brace and maintain gravity flow and air gap at the floor sinks.Permit not approved at this time.
