PIC instructed staff to close doors at time of inspection and stated that they would speak to the new manager about keeping doors closed. Discussed keeping doors closed/disabled at all times or installing air curtains. SECOND CONSECUTIVE VIOLATION Corrective Action: Corrected at time of inspection.

Final construction inspection for the smoothy bar. Construction complete and approved. Operations office will make sure that the following items ware addressed: 1). Mop sink will be caulked at the floor juncture.2). Coved base will be installed in the bar area including in the coves.3). At the mop sink an escutcheon plate will be installed for the AC condensate.4). In the restrooms, please touch up grout where missing, men' room schluter last stall, under counters around escutcheon plates.5). In unisex restroom, the hand wash sink drain is leaking.6). Please provide tempered water at the hand wash sink in unisex bathroom.Roll up doors at the end of the room will be disabled.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2.
