Observed container of macaroni noodles dated 1/11 at 45*F and container of gravy dated 1/11 ay 44*F at the 3 door sandwich prep refrigerator. Cold hold of food to be 41*F or lower. Items were embargoed and discarded. Advised staff to verify cold hold temperatures using metal stem thermometer. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with operator. Construction completed and approved.Inspection report emailed to Mario.Please address the following prior to routine inspection:1. Reseal the hood vent using heat resistant silicone as discussed (silver – regular clear silicone was used).2. Seal the gap between air duct and vent for water heater in warewash room.3. Seal the syrup line penetrations at BIB.4. Secure uni-struts for ice machine to the floor.Of Note:Discussed adding sticky flytraps for entry ways inside the kitchen for extra protection if rollup doors are used. Fly fan for dish wash room may be turned off while doors are closed. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Pre-final walk through. Construction is about complete. At the time of final please make sure:1). The hoods are caulked in. Both at the back wall and vertical junctures. Thermal silicone or silver caulk will work.2). Please turn on the refrigeration equipment in the kitchen that is not on and make sure the units hold at proper temperatures.3). A fly fan will be installed between the dish room and the bar, unit will not have an override and be tied in directly to the breaker box. Aim louvers 2%-3% outward and installation may be on the inside or outside.4). Please provide a hand wash sink in ;line of sight of the ice machine and within 20-25 feet. The best option is to install next to the dish wash pre-wash station.5). Some wood under the bar will be painted and sealed.6). At the first jockey box in the bar, please lift the drain lines off the floor and provide a 1 inch air gap.7). Provide the test report for the reduced pressure zone upstream of the soda system on site.8). Please caulk around the escutcheon plate of the to be installed hand sink.9). Please touch up the grout to the left of the mop sink at the base of the wall.10). Gaskets will be installed on the elisaon door between the kitchen and the hall way with restrooms.No County legal action will result from this inspection.Printed on site.
