Observed a large collection of fruit flies in the back kitchen area across from the 3-comparment sink on the walls. Discussed with person in charge to reach out to pest control services and get targeted treatment for the fruit flies. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Corrective Action: Corrected at time of inspection.

Inspector's notes
Observed a kitchen knife stored under a hot holding unit and the table that the unit was sitting on. Person in charge removed the knife from the area and moved it to the dish area to be washed, rinsed and sanitized. The new knife was stored in a clean, dry location. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

Observed some dishes on the clean dishes rack that were stored together with water still trapped between the containers. Person in charge separated the containers so that they can fully air-dry. Discussed with person in charge to fully dry the dishes prior to nesting them. All equipment and food contact surfaces must be air-dried after properly cleaning and sanitizing. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Repair the drain of the hand sink at the drive through area. 3. Repair the hand sink faucet for the cook line, prep sink faucet, and mop sink faucet (all have small leaks when in use). 4. Provide a uni-strut for the ice machine drain lines so they maintain 1-inch air gap with the floor sink. 5. Provide a rigid drain line for the soda carbonator RPZ to HUB drain.
