Quaternary sanitizer must have a concentration between 150ppm-400ppm to e used on food contact surfaces. Establishment was not actively washing food contact surfaces at time of inspection. Discussed with person in charge to have dispenser serviced and to dilute solution prior to use.

Discussed with PIC that they will need to use the chlorine packets and tests strips for sanitizing dishes and making sanitizing buckets. PIC agreed to use the packets until the dispenser is fixed.

Observed no hand towels available at the back hand sink. Informed PIC that all hand sinks need to be provided with hand drying provisions. Hand towels were provided at time of inspection. Suggested installing a towel dispenser for increased ease of use. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided hand drying provision for handwash sink at time of inspection. Observed the sanitizer dispensing tube to be leaking at the air gap portion. Informed PIC that it needs to be serviced and repaired. Observed the handle of the faucet at the three-compartment to be leaking when water is one and faucet is turned off. Informed PIC that the faucet needs to be brought back into good repair. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Inspector's notes
Observed broccoli cheddar soup and meatballs in marinara sauce on the hot holding line with internal temperatures ranging 120*F-130*F. Informed PIC that hot held food must be held at 135*F or higher. After investigation, it was found that the knob for the machine was bumped and turned off as a result. Instructed PIC to reheat the food to 165*F and place them back in the hot holding unit. The unit was turned on and properly hot holding at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. All TCS foods improperly hot held were reheated to 165°F at time of inspection. Corrective Action: Corrected at time of inspection.

E&D, class 3 10+OC inspection was conducted with the manager, Jennifer T. The establishment is open and operating.Permit is approved.Please address the following items prior to the re-inspection:1. Repair/ replace the evaporator pan inside the walk-in freezer. Condensation ice is forming on the ceiling of the unit and condensation liquid is dripping inside the unit. Noted there is an attempt to drain out the condensation tray but condensation ice is still forming due to drainpipe is not able to route the liquid to the floor sink outside of the unit. Recommend replacing the evaporator pan, wrap the condensation pipe with insulation to prevent discharging liquid frozen inside the pipe.2. Provide a hand sink with side splash guards at the food prep sink / 3-comp sink area. Sink must have hot and cold water.3. Certify the RPZ of the soda carbonator.4. Replace the non-testable backflow device vented dual check valve of the tea brewer if it has not been replaced within a year. 5. Check for leakage from all of the soda lines or pipes under the soda fountain station. There is a pool of brownish color liquid underneath the soda carbonator.6. Certify all testable backflow preventers and provide reports.7. Provide/ replace escutcheon plates around pipe penetrations. Some of them are bent and not cover the pipe penetrations. 8. Provide rodent seal around exterior pipe penetrations.9. Repair / replace the wall panel of the mop sink where the condensation pipe came through. Wall was not properly sealed, and water got inside the wall and this wall is bulging out.10. Replace missing cove base tiles around the floor sink of the soda fountain station. 11.
