Observed no hand cleanser at the hand wash sink near the 3 compartment sink. PIC supplied hand soap at the hand wash sink at time of inspection. Each handwashing sink must be provided with a supply of hand cleaning liquid, powder, or bar soap. Corrective Action: Corrected at time of inspection.

Observed establishment without test strips for chlorine to test the dish machine. Proper test kit must be present to measure chemical sanitizer concentration for re-inspection, or in lieu of re-inspection adequate proof of correction may be provided via electronic means. A test kit or other device that accurately measures the concentration of sanitizing solutions must be provided at all times. Corrective Action: Correct prior to re-inspection 2/12/2026.

Observed chemcial dispenser down stream of an AVB on the mop sink in the back. Employee was unable to disconnect at the time of inspection. Have hose disconnected prior to re-inspection on Monday, August 18, 2025. Chemical dispenser is permitted on dedicated water line. Corrective Action: Correct prior to re-inspection.

Observed burritos in the reach in refrigerator in the back without a date mark. Per PIC, items are made in a central kitchen and usually made and sent the same day as delivery. Burritos were delivered the day prior to inspection. Discussed dating the container to ensure that burritos are held only for the prep day plus 6. PIC had employee date container. Corrective Action: Corrected at time of inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov.
