Observed multiple hand sinks with no hot water supply. Water heater supplying hot water to hand sinks is inoperational. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained accessible in a condition that promotes handwashing. Corrective Action: Correct prior to re-inspection on 2/6/2026.

The person in charge has provided availability of both chlorine sanitizer and quat-sanitizer test strips via electronic means. The person in charge has also confirmed utensils found in state of disrepair have been removed from service and discarded. The person in charge stated physical facilities repairs have also been scheduled for completion.

Discussed placing lids or covering containers after use to prevent contamination of food from environmental factors. Observed a pump spray bottle with clear liquid and no label stored on top of a food prep table next to the proofers. The person in charge believed the liquid to be water but could not confirm the contents of the liquid. Discussed labeling all working containers with common name and using only food safe containers if storing food ingredient used for consumption.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.
