Inspector's notes
Observed a container of Canadian bacon prepared in the establishment with a brine that contains sodium nitrate. Nitrate salt usage is not approved without a variance. A variance/HACCP must be obtained prior to conducting any specialized processes. Person in charge (PIC) voluntarily discarded the product during the inspection to correct. Variance/HACCP applications are available through the customer portal "Permit Center" at https://maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Additional resources can be found at https://www.maricopa.gov/3978/Food-Variances. Corrective Action: Correct prior to re-inspection. Re-inspection is tentatively scheduled for 7/11/2025.

Establishment adds hemp seeds, hemp-based plant protein powder, and maca root extract powder to smoothies. Any substance(s) added to food must be GRAS (Generally Recognized as Safe) and/or used consistent with all legal requirements. The permit holder is responsible for ensuring that their menu items comply. Education was provided on the addition of food additives and dietary supplements during the inspection. Onsite visit conducted to assess patio as establishment has applied for a Dog Friendly Patio. Upon discussing with PIC that dogs are not allowed at tables within 7 feet of any door to establishment, PIC may decide to want to only allow dogs on upper deck of front patio with fireplace. Discussed that ice/water/dishware may not be kept outside to prevent contamination from hair. Provided PIC with dog friendly patio guidelines. Establishment has printed sign, discussed with PIC that sign must be posted on entrance to restaurant and entrance to patio. Patio has separate entrance apart from inside restaurant. The following items must be addressed prior to approval:1. Obtain dog safe chemicals for cleaning of patio.2. Water seal patio flooring. Contact inspector or department once completed for re-assessment.Dog Friendly Patio is not approved at this time. Only service dogs are allowed on patio at this time. Violations may be assessed in the future if non-service dogs are observed on un-approved patio. No County legal action will result from this inspection.

Observed past shellstock tags from within the last 90 days noted with a date written in sharpie pen with no shellstock from batches left to sell. Upon asking PIC and seafood employee, it was stated that written date represents day that shellstock was received. Discussed with PIC that shellstock tags must be noted with date of last sold piece of shellfish for traceback purposes. Mark shellstock tags with date last sold going forward. A reinspection will be conductd on Thursday, 10/12 or PIC may send photo of tags with last sold on date in lieu of onsite reinspection. Observed cooked rotisserie chickens hot holding in hot box next to blast chiller in kitchen at 114-118*F. PIC stated chickens had been in unit for less than 1 hour. PIC had employee re-heat chicken to 165*F before hot holding again. Hot held TCS items must be kept at or above 135*F to prevent growth of pathogens. Correct By: Corrected At Time Of Inspection
