Observed a large accumulation of drain flies in the back area near the 3-compartment sink and mop sink. Discussed with person in charge to seek pest control services from a licensed pest control applicator. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist. Observed a heavy accumulation of dust and debris on the ceiling vents and tiles above the dining area and also in the back area above the yogurt dispensers and above the food prep table. Discussed with person in charge to clean the ceiling tiles and ceiling vents so that they are free of dirt and debris and to clean them on a regular basis to minimize the accumulation of dust and debris on the ceiling tiles and vents. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed a section of the wall that the mop sink metal dividing wall is attached to (under the 3-compartment sink) and there is a gap/penetration into the wall space behind. It appears that some silicone/caulking that was there previously has degraded over time, creating a gap. Discussed with person in charge to repair the wall section near the mop sink so there is no longer an opening into the wall space behind the mop sink. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed significant accumulation of black/ grey dust on ceiling tiles and around vents in dining area and over front desk. Corrective Action: Correct prior to next routine inspection. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed stainless steel wall inserts around yogurt dispensers. From the back side, there are gaps and exposed surfaces of interior of wall. Caulking has spearated and exposed gaps and rough surfaces. Seal all areas to eliminate gaps and provide a smooth, non-absorbent surface. Corrective Action: Correct prior to next routine inspection.

Observed chlorine sanitizer test strips not available for the 3 compartment sink. Inspector used personal test strips to verify that the concentration for the sanitizer in the 3-compartment sink was in the appropriate range of 50ppm-100ppm for the chlorine based sanitizer. Instructed person in charge to acquire the appropriate test strips for their 3-compartment sink. Discussed with person in charge to obtain chlorine sanitizer test strips prior to reinspection.
