*3rd consecutive violation (1) Observed doughnuts being sold in the establishment with a QR code instead of a label. Person in charge (PIC) printed label at time of inspection. Label on food items should include the following information: (1) name of product, (2) processor name, (3) address or phone number, (4) ingredients, (5) weight, (6) be in English. (2) Observed bagged ice being bagged and sold in the establishment, but label lacks a store name/address/phone numbers. PIC stated employee must've forgotten to place the sticker on the ice bags. PIC corrected at time of inspection. Corrective Action: Corrected at time of inspection.

NonFOOD-CONTACT SURFACES of EQUIPMENT that are exposed to splash, spillage, or other FOOD soiling or that require frequent cleaning shall be constructed of a CORROSION-RESISTANT, nonabsorbent, and SMOOTH material. Establishment bags own ice during summer months. In freezer, bags of ice without proper identification labels. Identification labels were provided at time of inspection. Bulk foods available to the consumer shall be labeled to meet FDA food labeling guidelines. If made from two or more ingredients, a list of ingredients and sub-ingredients in descending order of predominance by weight, including a declaration of artificial colors, artificial flavors and chemical preservatives, if contained in the food shall be available for consumers to review.
