Observed a wrapped bag of lids on the floor of the dry storage area by the walk-in cooler. Person in Charge removed the lids and placed them on a shelf at least six inches above the floor. Equipment, Utensils, Linens and Single-Service and Single-Use Articles must be stored in a clean, dry location; where they are not exposed to splash, dust, or other contamination; and at least 15 cm (6 inches) above the floor. Corrective Action: Corrected at time of inspection.

Observed water leaking from condenser unit of the large Reach in Cooler. Cooler temped at 36*F and was otherwise in working order. Food was moved another working and in good repair cooler to prevent possible contamination from water. Equipment shall be maintained in a state of repair and condition to prevent cross contamination and foodborne illnesses. Corrective Action: Correct prior to next routine inspection.

Observed a nozzle and hose attached to the mop sink with water on. Person in Charge turned off the water and removed the nozzle. All plumbing must be maintained in good repair at all times. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from vacuum breakers. A long-term intervention session was offered to the Person in Charge. The Person in Charge declined. Corrective Action: Corrected at time of inspection. Second Consecutive Violation.
