Observed AVB missing cap at the mop sink in the back of the establishment in chemical storage area, two employee womens restroom sinks clogged, faucet leaking at three comp sink in produce cutting area when turned on, and toilet leaking in employee mens restroom from the base of the flush valve. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. *Consecutive repeat violation: 2nd.*
