Corrective Action: Corrected at time of inspection. Employee handled bag of raw chicken, then touched the handle to the walk-in, squeegee handle, mop handle and wet floor sign without washing hands. Had employee wash hands and surfaces were sanitized. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks.

Corrective Action: Corrected at time of inspection. Raw chicken, under temperature control per manager, piled above the rim of the pan at the breading station at 48*F internally. Manager opted to discard the chicken. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below.

In the reach in cooler near the rethermalizer, observed TCS foods including shredded cheese, cheesecakes, coleslaw and bacon with internal temperatures measuring 52-55*F with a probe thermometer. Observed the cooler door ajar approximately 1 inch. PIC stated the door is in disrepair and sometimes cracks open if shut quickly. Observed ambient air temp at 39*F with a min/max thermometer when the door was tightly closed. All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Advised PIC place a work order on the door for repair to ensure TCS foods are maintained at 41*F or below. Corrective Action: Corrected at time of inspection.

Observed large sections of flooring that have become heavily eroded and are no longer smooth and easily cleanable. Observed exposed aggregate rock worn through on the concrete. Also observed section of floor to wall juncture water damaged and in disrepair. Repair/replace to restore a smooth and easily cleanable surface. Discussed that if the areas is grounded down into smoothed concrete that the area must be sealed every 6 months. PIC stated the foods were thawed in room temp water about 1 hour prior to inspection. Observed the foods left unmonitored by employees. Observed RTE rice holding between 103-105*F in a covered plastic container on the prep counter near the rethermalizer. PIC stated the rice is soaked for 20 minutes in hot water before transferring to the walk in cooler. Per timestamp, the rice had been soaking for an hour unattended due to a business rush. Advised PIC to actively monitor foods while in prep to minimize time spent in the danger zone. All TCS foods cooling were uncovered to allow for rapid cooling and placed in the walk in cooler. Corrective Action: Corrected at time of inspection.

Also observed drain lines in disrepair under the 3 comp sink and food prep sink so grey water runs directly onto the floor and into nearby floor trenches. PIC stated they were aware the rethermalizer was in disrepair and were using the still water to remove the chill from frozen ROP bags of food such as red beans, mac and cheese, and gravy. PIC stated the bags are placed in the still water bath until they are no longer frozen, moved to the water bath in the table top steam table and then microwaved before being held hot in the steam table. Advised PIC to repair steamer prior to re-inspection on 1/12/2024 or provide an effective alternative to allow for proper reheating of TCS foods in 2 hours or less. Discussed that when reheating TCS foods that are commercially processed and in an intact package for hot holding, an internal temperature of at least 135*F must be reached within 2 hours and maintained for at least 15 seconds before the food can be hot held at 135*F. Advised PIC to thaw foods in the walk in cooler overnight to reduce the time needed to reheat. Advised PIC reheat in the oven, microwave (following approved procedures) or in a dedicated steam table capable of reheating ROP bags in 2 hours or less. Correct By: Correct Prior To Reinspection
Repeat Violation PRIORITY VIOLATION-5-202.11 (A), P: Approved System---Observed a shut-off valve attached to the mop sink faucet downstream from the AVB (Atmospheric Vacuum Breaker). Discussed with PIC that a shut-off valves down stream of an AVB is an unapproved set-up which may cause the AVB to fail. PIC stated the valve was placed downstream to be used as a faucet because the mop sink cannot be fully turned off due to a significant leak. The valve could not be disconnected at the time of inspection because the leak was too severe. A re-inspection will take place on 1/5/2024 to ensure the faucet has been repaired and the shutoff has been removed. PIC stated that the floors were re-done with two different types of concrete and coated with epoxy which wore off after two weeks. PIC reports the epoxy was given approximately 10 hours to dry after being applied. Observed most areas have become heavily eroded and are no longer smooth and easily cleanable. Observed exposed aggregate rock worn through on the concrete. Repair/replace to restore a smooth and easily cleanable surface prior to the next routine inspection. Discussed that if the areas is grounded down into smoothed concrete that the area must be sealed every 6 months. *fourth repeat violation* Core-6-501.12, C: Cleaning, Frequency and Restrictions ------ Observed standing grey water and food debris in the floor sinks near the front hand sink and drive-thru window. PIC was able to clear drains which were clogged due to heavy food debris accumulation in the filters. After clearing the filters, both drains cleared and appear to be functioning properly.
