Observed chemical dispenser connected to the mop sink faucet equip with an Atmospheric vacuum breaker. Chemical dispenser was observed connected to mop sink faucet via y-valve during inspection. Installing a Y-valve, similar downstream shut-off device, or obstruction places back pressure on the AVB. Both sides of a Y-valve connected to a sink for a chemical dispenser are pressurized; one side supplying water to the chemical feeder and the otherside to the shut-off valve on the Y-valve, or to a hose attached. Since, AVB’s could be kept on under constant pressure at all times, they will fail under such installations. When the faucet is used with a hose on the open side of a Y-valve, there is now a potential cross-connection hazard. Instructed PIC to remove Y-valve. Chemical Dispenser and Y-Valve (Wye) Installations guidance document sent electronically. Corrective Action: Corrected at time of inspection.

Predefined Comment: Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. Corrective Action: Corrected at time of inspection.

Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. After discussion with the person in charge it was determined the chicken wings were placed in the unit around 8AM. The chicken wings were discarded at time of inspection. After discussion with the person in charge it was determined all other TCS foods in the open top prep refrigerator were within safe time parameters. TCS foods in the open top prep refrigerator improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Corrective Action: Corrected at time of inspection.

Observed small cooler next to fryer with an ambient temperature of 57*F. Cooler contained raw sliced chicken and beef that were partially frozen and measured 32*F. The person in charge placed a work order for repairs at time of inspection and moved the proteins to another cooler. Discussed with the person in charge that acceptable methods of cooling include placing food in shallow pans, separating the food into smaller or thinner portions, using rapid cooling equipment, stirring the food in a container placed in an ice water bath, using containers that facilitate heat transfer, or adding ice as an ingredient. Spoke with store manage and district manager about current issues. District manager and store manager immediately took action at time of inspection. Observed grilled onions in the cold table on the line. With inspector probe thermometer the onions measured 54*F. The person in charge stated that they were made around 10am. Time of inspection was 2pm. The person in charge had a staff member move to freezer to bring down to 41*F at time of inspection. Discussed with the person in charge that when cooling time temperature controlled products they should be cooled from 135*F to 70* in the first 2 hours, then continuously cooled to 41*F or below in the following 4 hours.
