Observed the hand sink next to the frying station leaking drops of water onto the floor. Observed the hand sink in the ware washing area and, on the wall adjacent to the grilling station draining very slowly. Discussed bringing these pieces of plumbing back into good repair with PIC. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Observed pizza sauce with prep date of 1/13 and crushed tomatoes with prep date of 1/13 in walk-in cooler. Also observed cooked potato skins and cooked chicken strips in prep coolers with no date marking. PIC was unsure of when they were prepared. All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Reviewed date marking with items prepped/frozen/and thawed. Corrective Action: Corrected at time of inspection.

Observed clean paper towel and toilet paper rolls being stored inside the garbage receptacle cabinet in the men's and women's rest rooms. The rolls were touching the side of the garbage bin and in one if the cabinets, a plunger. There were used paper towels on the clean rolls in the men's room. Informed PIC that the clean rolls cannot be stored in these cabinet next to the trash cans and plunger due to contamination risk. PIC removed the paper towel and toilet paper rolls from the cabinets and threw them out. PIC stated that they will inform the janitorial staff. Single use articles cannot be stored in rest rooms, garbage rooms, under sewage lines, or any other source of contamination. Corrective Action: Corrected at time of inspection.

Observed standing water underneath the ice machines on exposed concrete flooring. The water is coming from the drainpipes of the ice machine and fallen ice and is also pooling under the rubber grid floor mats. Observed ice machine feet and a metal component on the ground heavily rusted. Informed PIC that the water needs to be cleaned up frequently enough to prevent organic matter growth and/or pest attraction. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Informed PIC that this pipe needs to be lifted so that it is slanting downwards and cannot collect water. Observed the left most drainpipe underneath the ice machines, draining onto the exposed concrete floor. Informed PIC that the pipe needs to be moved over or slanted so that the water drains directly into the floor drain. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Explained to PIC that these surfaces are not easily cleanable and need to be repaired. Concrete flooring is not allowed in kitchens because it cannot be cleaned properly, due to its roughness and porousness. Please repair with flooring that is smooth, non-absorbent, and easily cleanable. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection.
