Inspector's notes
Observed multiple deficiencies located at the mop sink: 1) Chemical dispenser connected to the main faucet equipped with an atmospheric vacuum breaker (AVB) backflow prevention device 2) Use of y-valve connected at the faucet equipped with an AVB 3) When turning the chemical dispenser into the on position, observed water leaking from the AVB device indicating inadequate functionality of the AVB device. The establishment shall refrain from connecting a chemical dispenser directly to the main faucet at the mop sink, refrain from using a y-valve at the mop sink, and replace the AVB back flow prevention device as it is considered a non-testable backflow prevention device and has been subjected to continuous pressure. Discussed the public health significance of these deficiencies with the manager including potential cross contamination between chemicals at the dispenser at the potable water source from the faucet due to failure of the AVB's functionality. Corrective Action: Correct prior to next routine inspection.
