Observed mop sink faucet equipped with atmospheric vacuum breaker (AVB) connected directly via hose to a chemical dispenser. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Person in charge disconnected hose at time of inspection. Corrective Action: Corrected at time of inspection.

Core-3-304.14, C: Wiping Cloths, Use Limitation Observed wet cloth in tray labeled sanitizer with 0ppm quat sanitizer solution. The person in charge discarded the wet cloth and solution and made a new solution with 150-400 ppm quat sanitizer. Discussed maintaining proper sanitizer concentrations.
