Discussed to update all online, electric, take out and physical menus. Items that require disclosure symbol are sunshine bowl, daily burrito, chipotle burrito, all omelettes (soft cooked) and Eddie's breakfast tacos. Inspector's notes

Observed severe liquid icicles on the ceiling in the freezer walk-in also observed the liquid to be frozen covering over several sheet trays from the top shelf. Observed the sheet trays to be uncooked cookies and they were not protected with any covering and had freezer icicles all over. Observed contamination and embargoed the cookies. Person in charge (PIC) voluntarily discarded the cookies at the time of inspection. Discussed with PIC nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Also discussed that food that is not going through the cooling process shall be protected from contamination. Corrective Action: Corrected at time of inspection.

Observed cooked onions with temperatures between 116 to 119*F in a steam well on the line at time of inspection. Observed steam well turned off at time of inspection. Per chef, items were reheated less than 2 hours prior to inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. All TCS foods improperly hot held were reheated to 165°F at time of inspection. Corrective Action: Corrected at time of inspection.
