Inspector's notes

Observed a dead cockroach under a table in the dining room and another one on near the bar area. Also observed a cup of yellow liquid that an employee stated was to attract and collect fruit flies. Discussed with person in charge (PIC) that they need to schedule an additional pest control visit to eliminate the pests during the hotter months. PIC stated that the proper lighted fly traps have been ordered by pest control and are scheduled to be installed. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist. Corrective Action: Correct prior to next routine inspection.

Observed in the bar-dining area there is carpet under the flooring. There are a couple of areas (near fireplace, near exit) where the carpet fibers are sticking out. All flooring must be installed properly and maintained in a way that the floors are smooth and easily cleanable. There is risk that moisture can get under the flooring and there would not be a way to clean it properly. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed ice being scooped with a shovel into a cooler. The ice being scooped is used for consumption. The employee was stopped and asked to fill their cooler with ice from the other ice machine that is not for human consumption. All ice must be in good condition, safe, and unadulterated. All adulterated ice was discarded at time of inspection. Discussed with person in charge (PIC) that a bigger sign should be made to help remind employee which ice machine is for coolers and which ice machine is for human consumption. Corrective Action: Corrected at time of inspection.
