Observed chemical dispenser connected to mop sink with atmospheric breaker valve through a y-valve. PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Discussed supplying chemical dispenser with the supply line for other chemical dispensers. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Inspector's notes

Observed containers of swiss cheese, provolone cheese, pre portioned deli meats, precooked bacon, sausage and chicken salad at a temperature of 51 F inside of cold top cooler located near the standing fridge. Per PIC, items were placed in the cooler at 7:30 am. Also observed butter on the counter located near conveyor sandwich oven. PIC stated that butter is kept out for around 2 hrs before being disposed of. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Corrective Action: Corrected at time of inspection.

Observed dairy yoghurt, cut watermelon and potato salad with no datemarking in standing reach-in unit located in warewashing area. PIC stated items were prepped on 10/21 and datemarked items at time of inspection. Observed establishment cooling multiple food items (chicken caesar salad, cut tomatoes, raw cookie batter with eggs) in containers double-wrapped with Saran (trapping heat). All TCS foods cooling were uncovered to allow for rapid cooling. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. PRIORITY VIOLATION-4-501.114, P: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Observed a bucket with sanitizing solution that was measured at 0 ppm quat and proceeded to measure the sanitizer concentration in the sanitizer compartment which also measured 0 ppm. The PIC called Ecolab and the aspirator was found to be faulty and was replaced by Ecolab and the violation was corrected at the time of inspection. Verify the sanitizer concentration with quat test strips when a sanitizing solution is made and provide training with staff on when and how to perform testing of the sanitizer concentration.
