Shut-off valve (chemical dispenser) observed downstream of atmospheric vacuum break (AVB) at time of inspection. Person in Charge (PIC) removed shut-off downstream from atmospheric vacuum breaker while onsite. Shut-off valves, including chemical dispensers, sprayer nozzles or y-valves, are not approved for continuous installation downstream from atmospheric vacuum breakers. All plumbing must be maintained in good repair at all times. Will follow up with management on long term solution for violation. THIRD CONSECUTIVE VIOLATION.
