Observed three separate top doors of the main prep table to be warped, broken, or otherwise in disrepair. All TCS food items being stored within were at or below 41'F at time of inspection. PIC stated that a work order has been placed to address the issue. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
