PRIORITY VIOLATION-5-205.12 (A), P: Prohibiting a Cross Connection Observed Y valve closed connected to mop sink with a chemical dispenser line attached. Discussed acceptable plumbing fixtures, operations and maintenance. Instructed operator to remove Y valve completely and detach chemical dispenser when not in-use.
