Observed establishment using high temperature dish machine. Establishment does not possess an irreversible measuring device to ensure that food contact surfaces are being properly sanitized. Informed PIC that an irreversible measuring device is required to be obtained prior to reinspection on 5/22/2026. Sanitizing using high temperature ware washing machines requires an irreversible measuring device to ensure food contact surfaces reach 160°F. Provide approved irreversible measuring device prior to re-inspection. Corrective Action: Correct prior to re-inspection on 5/22/2026.

Inspector's notes
Observed the larger of the two RPZs near the ice machines leaking and spraying water onto the floor beneath about every 10-15 seconds at time of inspection. Informed PIC that the RPZ is in disrepair and needs to be brought back into good repair prior to reinspection on 5/18/2026. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to re-inspection in 5/18/2026. 5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed the drainpipe of the spray nozzle sink fallen below the flood rim of the floor drain. Informed PIC that there needs to be an airgap for at least one inch between the outlet of the pipe and the top of the floor drain. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Final Construction inspection conducted with site superintendent, Mickey and Brian. Manager, Mike was on site also.Construction completed and approved. Inspection report was emailed to Mickey.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.Establishment approved to open after obtaining C of O. Equipment inspection conducted with Mickey B.Construction is about 80% complete.Approved to stock walk-ins after security sweeps are conducted. Report will be emailed to Mickey.Prior to final:1. Provide Pressure Vacuum Breaker for faucet in Berry Divine if faucet does not maintain air gap. Faucet has AVB, but will require a PVB that can be under constant pressure. PVB must be installed 1 ft above supply. If wanting to hide backflow from customer view, then install RP below counter.2. Provide Pressure Vacuum Breaker for both water connections at ice cream machine. Faucet has AVB, but will require a PVB that can be under constant pressure. PVB must be installed 1 ft above supply. If wanting to hide backflow from customer view, then install RP below counter. An RP will suffice also.3. Provide ASSE 1022 dual check valves for all water fed drink machines (tea and coffee) through out all stands.4. Provide faucets on sprayer handle at both 3 compartment sinks near large dish machine. Faucet must be available to fill all 3 compartments. Sprayer does not suffice to fill faucet.5. Ensure 3 compartment sink can drain. Drains were not rising enough to allow water to drain.6.
