Cooked diced beef being diced and placed into a 10" deep plastic tub. The tub was to be placed in the walk-in cooler for cooling at a later time. The meat was cooked around 7:40 or 8:00 AM today and was at 90*F at about 9:30 AM. PIC attempted to rapidly cool of the beef in the freezer but it was still at 80-88*F at 10:00 AM. PIC discarded the beef. Red salsa found in two deep covered plastic bucket-style containers. Both salsas had a prep date of 6/16 (confirmed by PIC). The salsas had not been brought out and had been cooling since yesterday. Both salsas had internal temperatures of 48-52*F. PIC discarded both salsas. Please ensure cooling time frames and temperatures are met. Provided cooling time/temperature and methods posters. Corrective Action: Corrected at time of inspection.

Final construction inspection. Construction complete and approved. Superintendent will address the items below before opening:1). Make sure the drain grate under the dining rom beverage machines is easily removable.2). Finish the grouting where missing in the hall way and restrooms.3). Please repair the three compartment sink drain that is leaking.4). For the owner, please provide new gaskets and make sure the three compartment sink valves are working.There are no beverage machines in place other than the soda station.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. Second inspection walk through. The following items are note for follow up:1). Please install a reduced pressure zone backflow valve upstream of the soda system at the water connection, Install with hub drain assembly and make sure the valve is tested and certified before final inspection.2). Please install an additional water line at the mop sink for future chemical dispensers.3). Install a condensate drain line off the water heater vent stack. Keep gravity flow and air gap.4). Install coved base along the front toe kick of the dining room beverage counter.5), Install escutcheon plates where missing, at gas lines (water heater and cooks line, drive through hand sink, water line to soda in drive through etc. Where missing.6). Install an additional faucet at the three compartment sink and provide a uni-strut for the spray arm assembly.7). Caulk/ silicone gaps where occurring, drive through shelve at window joint, east side walk-in gap FRP base tile, electric panels in the water heater room and storage room, etc.8). All fixtures will be caulked in after cleaning, sinks, toilets, hand sinks in restroom, also silicone the splash guards on the hand sinks at the top to the sinks and close gaps.9). Inside walk-in, caulk the gap around the reach in door frame at the walk-in wall juncture.10). Above the reach in door, put closure panel back in place and close gap. This is to the south of the main hood.11). Check all door sweeps and make sure there is no daylight.12). Fill in the gap between the tile and the back door threshold. Best to use caulk-grout of flexible material.13). If the front counter floor sink is not used install an expandable plug in the drain or trap seal so roached or sewer gasses can not come up when the p-trap dries.14). If the tea brewer is not part of the coke system and is connected to the water supply then make sure to install a dual check valve on that water supply that meets plumbing standard ASSE 1022.Store is at 85% Completion. Sinks plumbing will need to be completed. Make sure all plumbing is complete at final inspection. You have hot water at all sinks and all equipment is in place and holding temperatures.
