Predefined Comment: Due to the violations noted on this inspection report, County legal action may result from this inspection.Predefined Comment: Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 10 days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Observed 8 ft by 14 ft commercial walk in refrigerator installed in the back storage room of establishment. Per person in charge it was installed two months prior to current inspection. Walk in freezer is maintaing food frozen. Frozen foods include vegetables, fish, chicken. Also observed room in the back of establishment with meat cutting saw, three compartment sink, and handwashing sink. Room was built by adding a wall against three equating walls and then adding a ceiling at a lower height than original ceiling. The room has unfinished wall coatings. Per person in charge the meat saw has not been utilized.

Observed 11 small sealed bags of muffin sized bread being sold on the retail shelf. Per person in charge the bread was made and packaged at home and then brought to the store to be sold as a retail food item. Person in charge who made the bread from home is not currently registered with cottage foods program for Arizona State. Discussed proper food sourcing with person in charge. All food must be obtained from an approved source as defined by law. All food from unapproved source was discarded at time of inspection. PRIORITY VIOLATION-3-201.11 (A-B) - Priority: Compliance with Food Law; Approved Source.
