Observed unauthorized employee enter kitchen from back door and fill personal cup with ice, without washing hands. Scoop was contaminated by staff inserting it into personal cup and was placed back on top of ice machine. Person in charge failed to stop employee from entering kitchen and handling food and contaminating food contact surfaces. Discussed handwashing, restrictions for kitchen access, and availability of break room to person in charge. Observed single door reach in freezer with an ambient temperature ranging 8 (top shelf)-26*F (Bottom shelf). Foods inside unit soft to touch. Person in charge to call technician and maintain proper organization for airflow. 4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Predefined Comment: All equipment must be maintained in good repair at all times. Repeat Violation Observed missing vent cover in pantry. Person in charge waiting for order to be received by maintenance. Wall at mop sink with small gap cut out for plumbing for AC. Partition also contains a large cut out causing hole and porous surfaces to be exposed. Maintenance to coordinate with management to close cut out. Inspector's notes

Scoop, spatulas, and ladles observed with food debris and oil while stored as clean inside drawers. Also observed food debris inside drawers where clean utensils are stores. PIC stated that they will clean drawers. PIC took the dirty utensils to the dish room to be properly washed, rinsed, sanitized, and air dried before returning to storage. Reviewed the requirement of keeping all food contact surfaces and dishes/utensils clean to sight and touch with PIC. Corrective Action: Corrected at time of inspection.
