Due to the violations noted on this inspection report, County legal action may result from this inspection.Observed the installation of a dish machine, which was also noted on the previous inspection report, dated 10/21/2025. Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.Also observed the installation of a new water line next to the coffee/tea brewers. The water line is a flexible hose that does not have an air gap. Please ensure that a backflow device (ASSE 1022) is installed on the water line. The faucet at the mop sink does not have an approved AVB installed. According to PIC, the faucet and mop sink area were recently replaced. Provided a picture of an approved faucet with an AVB, which must be installed. Discussed AVB's and backflow. Non-testable Anti-siphon Vacuum Breaker and hose bibb vacuum breakers are designed to protect potable water supplies from the backflow of non-health hazard cross-connections and low flow, non-continuous pressure installations.
