Predefined Comment: No County legal action will result from this inspection.Observed establishment was providing for retail sale packaged dessert cakes with the buisiness name "Heavenly Treats." The person in charge stated the dessert cakes are not made in the establishment and are purchased wholesale from an outside vendor. Upon further investigation, it was determined the food source for the dessert cakes was from a Arizona Cottage Food Registrant "Heavenly Treats CF-108398." Observed packaged dessert cakes did not meet full labeling requirements as required by law. Labels only showed buisiness name, ingredients, and date mark. Disscussed labels requirements:1. The product’s production date.2. The name and registration number of the food preparer.Registration numbers began on July 1, 2019. Home addresses are no longer required to be included on the label.3. A complete list of all ingredients used in the product, including those in its components.4. This exact statement: “This product was produced in a home kitchen that may come in contact with common food allergens and pet allergens and is not subject to public health inspection.”5. If applicable for the product, a statement that the product was made in a facility by individuals with developmental disabilities.6. The Department's Website: www.azdhs.gov/CottageFood.The person in charge removed the dessert cakes from retail sale at the time of inspection. Provided AZ Cottage Food resource information.

Inspector's notes

Observed cooked mashed potatoes with internal temperature betwee 53-55F cooling in the reach in cooler by the front counter. The mash potatoes were portioned in plastic container with closed lids and on the bottom shelf of the reach in cooler. The person in charge stated the mash potatoes had been placed for cooling less than 6 hours ago and moved the food items into the walk in cooler uncovered for rapid cooling.

Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-201.14 - Core: Floor Carpeting, Restrictions and Installation Addl Notes: Observed carpeted mat in the toilet room. Discussed non-approved floor coverings in food preparation areas, walk-in refrigerators, warewashing areas, and toilet room areas. The person in charge removed the carpeted mat at the time of inspection.

Predefined Comment: Due to the violations noted on this inspection report, County legal action may result from this inspection.Reviewed dating methods for RTE to go containers of salsa and chiles. ***Observed an outdoor walk-in refrigerator with boxes of raw chicken, raw pork and vegetables used for the establishment located on north side of restaurant behind the white gate. Person in charge states the walk-in was added about 2 months before inspection. Unit was temping at 39*F at time of inspection and all TCS foods were at 41*F or below.Predefined Comment: Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.

#21 — Proper Hot Holding TemperaturesPriority3-501.16

Inspector's notes

Observed cooked pork ribs in a hot insulated carrier underneath prep table at time of inspection. Ribs had an internal temperature of 115-120*F via probe thermometer at time of inspection. Explained to person in charge (PIC) that except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. All TCS foods improperly hot held were reheated to 165°F at time of inspection. Recommended removal of hot insulated carrier from establishment because the intended purpose is to not hold at 135*F or above for a long period of time. PIC will use standing electric hot holding cabinet to store pork ribs. Corrective Action: Corrected at time of inspection.
